The program assists with water leaks, provides education on watering, and advises on how to best use appliances that use water wisely. Steps to avoid running out of medication To avoid having problems getting your medication, you can do the following: ■Let the pharmacy know if the doctor changed your dosage. (The doctor should give you a new prescription.) ■Show your insurance card every time you pick up your medication. ■If you are going out of town, make sure you have enough medication before leaving. ■Try to refill your prescriptions before the weekend or any holidays. Most offices are closed on weekends and holidays, and it may be hard to reach your doctor, if needed. ■Some medications need authorization. If your doctor did not get the authorization, it may cause delays. Some medications that need authorization are: ■Growth hormones ■Pain medications (if narcotic) ■Antiseizure agents ■ADD / ADHD medications Your doctor can look up if your medication will need authorization. The medications covered under Medicaid / CHIP can be found on the “Drug Formulary” link on our website at www.elpasohealth.com/ subcontractor-information. Caregivers may assist with bathing, dressing, or taking medicine. Before letting anyone in, always ask to see their name badge or ID. If you’re unsure, call their agency or your El Paso Health Service Coordinator to confirm. Stay alert! If someone shows up without an appointment or refuses to show ID, do not let them in. You can report any concerns to El Paso Health or the home care agency. To help you stay safe, keep important phone numbers in an easy-to-reach place. 3 wellness Your health matters, and one of the easiest ways to stay safe is by keeping track of your medications and sharing that list with your doctor at every visit. Why does it matter? Your doctor needs to know everything you take, including: ■Prescription medications ■Over-the-counter medicines (like pain relievers or allergy pills) ■Vitamins or herbal supplements This helps your doctor: ■Make sure you don’t get 2 medicines that do the same thing. ■Avoid mix-ups that could make you sick. ■Check if the dose is right for your body and health. What can you do? ■Keep a list of your medications in your wallet or phone. ■Bring your medicine bottles or a photo of them to your visit. ■Update the list anytime your medications change. resources Be prepared: Make the most of your Service Coordination visit Your Service Coordinator’s job is to help you get the care you need. To do that, your Service Coordinator needs your help. One way to help is to be prepared for their visits. Here’s how: 1. Gather important items. ■Have a list of current providers, medications, and conditions available. ■Have a valid ID and insurance cards ready. 2. Make a list of questions you may have. ■Write down any health problems or symptoms you have. ■Think about what help you need, like personal care, rides to appointments, or changes to your home. ■Consider what you want to achieve with the help of your Service Coordinator. 3. Review your current care plan. ■Look at the services you are already getting, and note any changes you need. 4. Involve family members or caregivers. ■Ask a family member or caregiver to be there during the visit to help. 5. Be ready to talk about your daily routine. ■Be prepared to talk about your daily routine and any challenges you face with activities like bathing, dressing, and eating. 6. Prepare for an in-home assessment. ■Make sure your home is safe, and fix any hazards. ■Keep pets away and in a safe area during the visit. ■Make sure you are comfortable and have a quiet place to talk. 7. Follow up. ■Take notes during the visit to remember what was discussed. ■Understand what you need to do next, and follow up on any actions.
